What
Sometimes Claude Code needs permission and there is nobody to ask, so it refuses. The reason it records is now dont_ask_mode when the session is in don't-ask mode. In other cases it stays nobody_to_ask, the only reason recorded before.
Why
The recorded reason is now accurate, so a refusal caused by don't-ask mode can be told apart from one where no person was available.
