What
Claude Code keeps a list of files by name to pass along between turns, where a turn is one exchange of you writing and Claude answering. It already refreshed entries when two files had the same name. It now also refreshes entries whose uploaded file is no longer among the live files, meaning the upload was deleted.

Entries pointing to deleted uploads are cleaned up instead of lingering.
