What
Claude Code keeps a list of permission requests that are still waiting for an answer. Each request is about a tool call, meaning one use of a tool by Claude. Before, a request could stay on that list after its tool call was already answered, if the tool matched an extra rule. That exception is gone. Any request whose tool call has been answered is now dropped from the list.
Why
This may change which permission prompts are shown to you again, for example after a reconnect.
