What
In remote and web sessions, Claude Code tells the server when Claude is waiting for your permission, so the request can be shown to you. It now picks one waiting request to lead with. If the server refuses that record, Claude Code can send it again in a slimmed form, with oversized parts blanked out. A refusal after the session has closed is treated as dropped.
Before, the waiting request was sent once, with no recovery if it was refused.
Why
A large permission request could be rejected by the server and never appear, leaving the session waiting for an answer you could not see.
