4545* Added a warning in Cowork, Chat and Code once a person has used 75% of the token limit set with `inferenceMaxTokensPerWindow` and `inferenceTokenWindowHours`. Set `inferenceTokenWarningPercent` (1 to 100) to change the percent. At the limit it reads “Token limit reached” and cannot be closed.
4646* Changed `managedMcpServers` entries with `server: "github"` so that `pull_request_review_write` asks before every call, comment-only reviews and thread resolves included, even when `toolPolicy` allows it. Where no prompt can be shown, the call is refused. `toolPolicy` can still block it.
4747* Changed a failed `PreToolUse` or `PermissionRequest` hook with `"shell": "powershell"` to block the call on PCs without Git for Windows unless it sets `args` or `"onFailure": "continue"`. If written for Bash: remove `shell`, add `"onFailure": "block"` to keep blocking, install Git, reopen Claude.
