What
The instruction Claude receives after you approve a plan has been reworded. It now tells Claude that you may have edited the plan document. Claude must read the document before it starts working. Where the document differs from Claude's own draft, the document counts as your instruction, and any step you removed counts as removed on purpose.
Why
Claude should work from the plan as you left it, not from an older copy of it.
