What
Some tools cannot show you a permission prompt. Claude Code refuses those before they run when no prompt is raised and the safety check did not allow them. That rule now has extra conditions, so it applies in fewer situations, and some decisions marked as allowed now go through instead of being refused.
Why
This changes when a tool that cannot ask you is refused automatically.
