What
A tool result is what comes back to Claude after it uses a tool. Some tool results point at other tools by reference rather than spelling them out. A new step rewrites those references into a plain text block that lists the tools, for models that are not exempt. Claude Code logs when this happens.
Why
It is a fallback so that details of tools loaded on demand still reach models or services that cannot read tool references.
