What
When the set of tools available to Claude changes during a session, including tools from MCP servers (outside services connected to Claude Code), Claude Code sends Claude a note about it. That note now has a separate section for tools that were replaced, split apart from tools that were already shown earlier. Before, the same information was ordered differently.

The reminder Claude receives about tool changes is worded and arranged slightly differently.
