Unclear The setting that switches this prompt on is not named, so whether it applies to you is unclear.

What

MCP servers are outside services connected to Claude Code that give Claude extra tools. Claude Code now has an approval prompt for when Claude uploads a file to one of these services by pointing at the file. It reads:

Claude wants to upload a file to (service name). This upload needs your approval even if you already allowed this tool.

The prompt appears even if a permission rule already allows the tool. It sits behind a setting that is off unless it is switched on remotely.

Why

Where this applies, an allow rule for a tool is no longer enough for it to send one of your files to an outside service. You are asked each time.
