Published model list can now be held back to Claude Code's built-in behaviour

Claude Code can load a published list of models from Anthropic instead of relying only on the one built into it. It now has two more reasons to refuse that published list:

• The publisher holds a provider on Claude Code's built-in behaviour.
• The built-in starting copy of the list would move a model to something else.

When Claude Code is started against a third-party provider, it now puts the list off when nothing is saved locally, instead of fetching it.
