When an action on an artifact fails because Claude Code's information about it is stale, the error message has new wording. An artifact is an item of content that Claude Code works with.
Before, the message said the artifact had changed while it was being reviewed.
Now, it says that what the tool knew of the artifact was missing or out of date.
Why
You see this message yourself. The new wording is more accurate because it also covers the case where the information was never there, not just the case where it changed.
