What
Claude Code can send permission questions (asking whether it may run a tool) to a channel outside the terminal so you can answer from there. Each question has a request ID.
If the same request ID is drawn twice in one session, Claude Code now ignores any channel replies that carry that ID. A warning tells you to answer in the terminal instead.
Why
A repeated ID could let one reply be counted as the answer to a different question. Ignoring channel replies for that ID closes that gap, so in that case your approval has to come from the terminal.
