When Claude Code runs as a cloud session without the interactive screen, it uses a stall check that notices when the session seems stuck. That check can now be told when a permission request (a prompt asking you to allow an action) opens and when it closes.

Why

This is likely meant to stop a session from being treated as stuck while it is only waiting for you to answer a permission prompt.
