A tool call is a single action Claude takes, such as running a command or reading a file. Foreground calls are the ones you are currently waiting on. When Claude Code's scheduler decides to move work to the background, it now also tries to move any foreground tool calls that are still running. If moving one fails, that call stays in the foreground and Claude Code logs a note saying so.
Why
In practice, a slow action that is already running can be sent to the background instead of holding up the session until it finishes.
