Richer Tool-Call Labels in Status Display

AskUserQuestion prompts and Plan-mode submissions now show descriptive labels in the status display:

  • Questions show the header/question text (e.g., "Question: Which library?" or "Tap to answer")
  • Plan submissions show "Plan ready for review"
