#### When a machine's `apiKeyHelper` isn't in effect
The whole hunk
from line 153, old and new numbered

from line 153
153153* **First-party sign-in keys**: [`forceLoginOrgUUID`](https://code.claude.com/docs/en/settings-reference#forceloginorguuid) and the `"claudeai"` and `"console"` values of [`forceLoginMethod`](https://code.claude.com/docs/en/settings-reference#forceloginmethod). If you also deploy an MDM policy or managed settings file, set them there and in server-managed settings from claude.ai until every client runs v2.1.299 or later. For where to set the keys afterward, and which source's value applies, see [Which managed source supplies login keys](https://code.claude.com/docs/en/managed-settings.md#which-managed-source-supplies-login-keys).
154154* **Gateway sign-in keys**: Claude Code never reads [`forceLoginGatewayUrl`](https://code.claude.com/docs/en/settings-reference#forcelogingatewayurl), [`gatewayInternalNetworks`](https://code.claude.com/docs/en/settings-reference#gatewayinternalnetworks), or the `"gateway"` value of [`forceLoginMethod`](https://code.claude.com/docs/en/settings-reference#forceloginmethod) from server-managed settings, so a value there neither applies nor hides one set in an MDM policy or managed settings file. The [`managedSourcesBehavior` entry](https://code.claude.com/docs/en/settings-reference#managedsourcesbehavior) says which admin source on the machine supplies them.

156#### When a machine's `apiKeyHelper` isn't in effect
157
158When your server-managed settings are the selected source, an `apiKeyHelper` in a machine's managed settings file or MDM policy isn't in effect. Claude Code then [refuses prompts](https://code.claude.com/docs/en/errors.md#claude-code-didnt-send-your-claude-ai-login) in a session that would use a saved claude.ai login, provided both of these hold:
159
160* The file or policy turns on a cloud provider, or any source sets an API address other than Anthropic's
161* The server-managed settings set no helper and don't name Anthropic's API address
162
163To clear the refusal, keep the helper and its API address in effect, name Anthropic's API address in the server-managed settings, or remove the helper from the machine's policy. The refusal requires Claude Code v2.1.314 or later.

157166
158167Claude Code fetches settings from Anthropic's servers at startup and polls for updates hourly during active sessions.
No line in this hunk matches that.
