What
A tool call is Claude asking to use one of its tools, such as running a command. Sometimes you approve a call, but Claude Code then cannot get that tool from your computer, so it never runs. Claude Code now sends Claude a message that says exactly this: the call was allowed but did not run, Claude should call the tool again if the task still needs it, and it may need your approval again.
Why
This helps Claude recover when a tool that lives on your machine, for example one supplied by your editor, is briefly unavailable. Instead of assuming the call worked or giving up, Claude knows to try again, and you may be asked to approve it a second time.
