What
When you rename a session, or an agent is given a name, Claude Code saves that name into the session's transcript (the file that records the conversation). If writing to the transcript fails, the save step now reports the failure back. Before, the error was only written to a log and the save carried on as if nothing had happened.
Why
Other parts of Claude Code can now tell that a rename did not stick, rather than assuming it worked.
