What
Before you approve a change, Claude Code shows a preview of the file it affects. If that file cannot be read, the action is now refused and reported back as a tool error, saying the file could not be read to show the change for approval. Before, the failure was not caught and passed straight up.
Why
The model now sees a plain error and can respond to it, rather than the failure breaking the step.
