MCP servers are outside programs that give Claude Code extra tools. Claude Code used to pass Claude a short note saying "The server's tools should now be available." once a server's tools were added. That note is now empty, so Claude no longer receives it.
Why
After a server reconnects, Claude no longer gets this hint that the server's tools are ready to use.
