Plan mode is where Claude works out a plan and shows it to you for approval before making changes. ExitPlanMode is the tool Claude uses to leave plan mode and present the plan. A new version of its instructions makes the plan text optional, telling Claude to "Leave it out when plan mode's reminder has you keep the plan in a Claude Doc."
