What
Plan mode is a permission mode in which Claude works out a plan before making changes. When a permission update switches the session into plan mode from a different mode, Claude Code now also clears any plan document left from before. Previously only the mode changed and the old plan stayed.
Why
A fresh round of planning starts from a blank plan instead of picking up an old one that may no longer fit.
