Unclear Which conditions disable the option, and what label replaces it, are not stated.

What

The permission prompt is the dialog where Claude Code asks whether it may go ahead with an action. When this prompt is in its expanded view and a certain condition is not met, one of its approval options is now disabled. Its usual label is replaced by a fixed piece of text, and choosing it is refused. Before, that option could always be selected and always showed its own label.

Why

You may now see an approval option you cannot pick. If that happens, choose one of the other answers.
