What

A permission request is how an action asks for approval before it runs. There is a new error for when a server does not accept part of such a request. It is reported as an invalid request, says the request was not run, and ends with "Retrying won't help."

Why

Programs that talk to Claude Code remotely get a clear answer that trying again will not succeed, instead of retrying pointlessly.
