What

Claude Code keeps a copy of a permission policy for the session. A permission policy is a set of rules about what Claude may do. It now remembers whether that copy was loaded from disk without having been checked. If it is still unchecked when needed, Claude Code throws the copy away and loads it again. Before, it always used whatever copy it had.

Why

An out-of-date copy can now be refreshed once it becomes possible to check it, instead of staying in use for the rest of the session.
