Organisation-wide overage reports no longer include a limit scope

When you go past a rate limit, which is a cap on how much you can use in a period, Claude Code reports the overage. That report used to include which scope the limit applied to whenever one was known. It now leaves the scope out when the limit applies to the whole organisation.
