Teammates need lead approval to leave plan mode based on their task

In an agent team, several Claude agents called teammates work under a lead agent. Plan mode is a mode where an agent plans before it makes changes. When a teammate tries to leave plan mode, Claude Code now looks at that teammate's task:

• If the task cannot be found, plan mode is not exited, and the teammate is told: "This teammate's task could not be found, so plan mode was not exited. Ask the lead to approve your plan."
• If the task is still in plan mode, or the task requires a plan, the plan goes to the lead for approval.

Before, approval depended on a general check of whether the agent was in a team. A teammate leaving plan mode also no longer changes the shared tool permission mode.
