What
Claude Code reports usage data about what happened after you answered a permission prompt. That report is now either "applied" when the action was run again, or the reason it was not. The separate value for a tool that was not loaded is gone.
Why
Nothing changes in what you see; only the usage data Claude Code sends is reported differently.
