In plan mode Claude writes up a plan before making changes, and each plan has a short name used to identify it. Claude Code now checks that name, whether it is being set or read back from a session log. It must be text, no longer than a set maximum, and match an allowed pattern. A name that fails is refused and logged. The helpers that work out the plan folder, skills and memory locations also now cope safely with a missing value.
Why
This stops a plan name from being shaped like a file path, so it cannot point Claude Code somewhere it should not.
