Claude Code has a new reason for refusing to read a file: the file "changes what the child loads from this folder". A file like this can change what gets picked up from a folder when it is loaded.
The list of refusal reasons is also shorter. It used to have four: read_it_first, not_as_read, over_the_cap and refused. Now it has two: over_the_cap and refused.
