Proxy failure notes add a hint when a connection is refused

A proxy is a go-between server that network traffic passes through. When the agent proxy fails, Claude Code shows a note about it. The note can now include one extra line when at least one recorded failure was a refused connection and a further condition is met. The extra line appears before the line that begins "For details: curl". Before, there was no extra line for refused connections.
