What

When Claude Code asks for permission and that request ends without your answer, it tells you why. Those reasons have been reworded to refer to the approval directly. For example, a timeout now reads "nobody answered the approval within" a given time, and a replaced request now says a newer copy of the request replaced this one.

Why

The reasons are easier to understand, especially when approvals are handled remotely or by another app hosting Claude Code.
