MCP servers are outside programs that give Claude extra tools. A server can send a notice that its list of tools has changed. If that notice arrived before Claude Code had set up its listener for it, it used to do nothing. Claude Code now refreshes the server's tools in that case too.
Why
Tools that an MCP server adds or changes early in a session now show up, instead of Claude working from an out-of-date list.
