What
A tool call is a single action Claude Code takes, such as reading a file or running a command. When one finishes, Claude Code now adds a new marker to the result. That marker is also used when deciding whether the result counts as a denial, meaning the action was refused.
Before, a result counted as a denial only when it carried an explicit denial type. Now the new marker can make it count as one as well.
