When files staged for a session (copied in ahead of time) fail to land where expected, Claude Code now always records a warning in its log.
When a server-controlled setting is on, Claude Code also sends the model a quiet note saying how many files may not have arrived, and that those paths may be missing or may hold an older copy.
The model learns that files are missing, rather than silently reading old versions and working from them.
