Skills replaced during a session can be listed to Claude again

Claude Code now forgets it already announced a skill once that skill is replaced mid-session, so the new version can be listed again

What

A skill is a packaged set of instructions Claude Code can load. Claude Code keeps track of which skills it has already listed to the model in each session, or for each helper agent. Before it sends the skill list, it now removes any skills that have been replaced from that record. A skill swapped out mid-session can therefore be announced to the model again instead of being treated as already sent.

Why

In practice, the model can hear about an updated skill rather than relying on the version it was told about earlier.
