What
Plan mode is where Claude works out a plan before making any changes. When you resume a session, Claude Code can now re-enter plan mode if the saved mode says plan mode was on, or if the conversation shows plan mode was still open.
In remote mode, Claude Code also now reports to Anthropic when an answer went unrecorded.
Why
A session you resume can come back in plan mode rather than in the mode you might expect.
