What
MCP servers are add-ons that give Claude Code extra tools. If a server is removed, changed, disabled or refused while Claude Code is still connecting to it, the finished connection is no longer kept. It is marked as failed, with the message "The connection was not kept: the server was removed, changed, disabled or refused meanwhile".
Why
This avoids a session holding on to a connection built from settings that no longer apply.
