An old dialog response can no longer cancel a newer one

Cleanup of pending dialog requests now only removes an entry if it still belongs to the same request, fixing a race

Claude Code keeps a list of dialog requests that are still waiting for an answer. When a request finishes or is cancelled, its entry is now removed only if the entry still belongs to that same request. Before, finishing any request with the same identifier removed the entry, even if a newer request had replaced it in the meantime. The same check applies to both answering and cancelling.

Why

This stops a timing problem where a late answer to an older dialog could cancel or overwrite a newer dialog that reused its identifier.
