Hosted session claim and file staging requests adjusted internally

When a hosted session is taken over with claim_session, Claude Code builds a set of startup details. Those details are now worked out only when needed, and include whether the session hides some of what it saves, whether it is connected remotely, and who writes to the session. Separately, the request that stages a file no longer applies an extra condition about the mount path when marking staged output.
