Remote sessions send the refusal of a withdrawn permission request together with its cancellation

A permission request is the question Claude Code asks before taking an action that needs your approval. In remote sessions, when such a request is withdrawn, the worker answers it with Deny. It now holds that Deny answer until the matching cancellation is ready, then sends both to the server in a single request. Before, the two were sent separately.

If the worker shuts down while some of these Deny answers are still held, it logs how many were never sent.

Why

Sending both together stops the server from receiving the refusal and the cancellation at different moments, which could leave the two out of step in a remote session.
