Poll tool gives a clearer refusal message and records how each use ended

The Poll tool only works in the main conversation. When another agent tries to use it, the error now also carries the message "Poll refused for this agent".

Claude Code also now records how each use of the Poll tool ended, how long it took and how many events it saw. One recorded ending is a duplicate call.
