What

A monitor is a script that Claude Code runs and watches in the background. When a monitor finishes, fails or is stopped, Claude Code shows a short notification. Those notifications used to be worded separately at each point. Their text now comes from one shared place. Earlier examples of the wording include "ended without producing output", "stream ended" and "stopped".

Why

The wording of these monitor notifications may have changed, so a message you are used to seeing could read differently.
