What
A hook is a command you set up to run automatically at a set moment, such as when a session (one conversation with Claude Code) starts. Claude Code now places the messages from those start-up hooks into the conversation one at a time, and keeps track of which hook messages it is placing. Previously it rebuilt the whole conversation list with the hook messages put at the front.
Why
This is mostly internal, but it can change when hook messages appear in the conversation.
