What
Claude Code keeps a record of where it has seen, or ruled out, data marked as covered by HIPAA, the US health-privacy law. When an update rests on earlier evidence and the new content carries no such mark, the record is now left as it was. Before, the entry was always removed and then added back as seen or ruled out.
Why
