When Claude Code loads a saved conversation, a new step called regroupSplitResponses tidies the order of its rows:
Pieces of one reply from Claude that ended up separated by other rows are put back together.
A tool result (the output of an action Claude took) that was written before the action itself is moved to after it.
If regrouping would lose or duplicate a row, it is skipped and the conversation loads in the order it was saved.
Why
Resumed conversations should show replies and tool results in a sensible order. The safety check means this step never loses or duplicates part of your history.
