What
Plan mode is a mode where Claude works out a plan before it changes anything. When you leave plan mode and choose to clear the conversation so far, Claude Code now labels that clear with resetTrigger: "plan_mode_exit". The step that resets focus-mode memory also now receives a reason.
Why
The reset now records what caused it, so a clear that comes from leaving plan mode can be told apart from other clears.
