What
When a text file Claude has read is changed, Claude Code normally attaches a short excerpt of what changed. It now leaves that excerpt empty in three cases:
the earlier read of the file came back empty even though the file had content
the file's name matches a particular internal check
the overall limit for these excerpts has been reached
In each case Claude Code also updates its record of what it has read from that file.
Why
In these cases Claude gets a note that the file changed without the changed text, instead of an excerpt that may be misleading or too large.
