The effort-downgrade nudge now works per model instead of just one, but each model needs its own flag turned on.

The effort-medium nudge, previously hardcoded to a single model and level, is now driven by a per-model config table (v2). It currently covers claude-opus-5 (nudging from high) and claude-fable-5-1 (nudging from high, xhigh, or max), with each model gated by its own flag.
